My First Visit

Welcome to Channel Islands Oral & Maxillofacial Surgery! We are honored to have the opportunity to serve your surgical needs. Our highest priority is to provide you with excellent care.

We value your time and ask that you complete your Patient Registration and Medical History forms before your appointment. This will allow our doctors and staff adequate time to review your paperwork and ensure you and other scheduled patients are seen on time. If you cannot complete these forms online, we ask that you come to our office 30 minutes prior to your initial scheduled time to fill out paperwork. Otherwise, please arrive 15 minutes prior to your appointment time to check-in and have any necessary radiographs taken. If for any reason you cannot keep this appointment, please call our office at least 48 hours in advance. This will allow adequate time for us to contact other patients in need of care to fill the vacant time slot.

Patients under the age of 17 must be accompanied by a parent or legal guardian. If you are having sedation with your surgical procedure, do not eat or drink anything for eight (8) hours prior to your surgery. Prescription medications may be taken with a sip of water, unless directed by your surgeon. Please remove contact lenses before surgery.

A payment will be necessary at the time of your appointment, even if insurance coverage is expected. Due to the increase in surcharges, all credit card payments will be charged a 2% service fee.
